When importing items via the DDB importer, I’m seeing that that majority of icons and images used are broken image links. Looking into what image it’s trying to use, the file doesn’t exist:

Are you perhaps using the Iconizer module GitHub - VTTAssets/vtta-iconizer: Sets item's icons automatically based on a prepulated and/or user-defined dictionary ?
The paths set there point to the dnd5e system, but those icons have been moved out of the system paths some versions ago into core. The iconizer package will still attempt to use them, though.
If you are using it, you can fix the issue by disabling Iconizer and re-importing the items into your world from the ddb compendiums
